When an Ear Ache Needs Medical Attention

Ear infections are one of the most common reasons children visit a pediatric urgent care. Many ear aches are caused by middle ear infections (otitis media), often following a cold or upper respiratory illness. Younger children may not be able to describe the pain and instead become fussy, pull at their ear, or have trouble sleeping.

At KIDS Urgent Care, we evaluate children with ear pain, examine the ear with otoscopy, and help you decide on the right next step. Bring your child in if ear pain lasts more than a day or two, you see fluid draining from the ear, your child has a fever, or symptoms are not improving with home care.

Some symptoms point to a serious infection and need emergency care. Call 911 or go to the nearest emergency room for severe headache, stiff neck, confusion, swelling or redness behind the ear, persistent high fever, or signs that your child is very lethargic.

Common Signs of an Ear Infection

Symptoms can vary by age. In babies and toddlers, look for signs like:

  • tugging or pulling at one or both ears
  • increased fussiness or crying, especially when lying down
  • trouble sleeping
  • fever between 100°F and 104°F
  • loss of appetite
  • fluid draining from the ear (yellow, white, or blood-tinged)

Older children may also describe:

  • sharp or dull ear pain
  • a feeling of fullness or pressure in the ear
  • muffled hearing
  • balance issues or dizziness

If symptoms have not improved within 2 to 3 days, or if your child has severe pain or high fever, it is best to have them evaluated.

Frequently Asked Questions

What causes ear infections in children?

Most ear infections in children are caused by viruses or bacteria that build up fluid behind the eardrum, often following a cold, sinus infection, or upper respiratory illness. Young children are more prone to infections because of the shape of their eustachian tubes.

No. Current pediatric guidelines support a watchful waiting approach for some children with mild symptoms, especially those over 2 years old. Antibiotics are typically recommended for children 6 months and older with moderate to severe symptoms, fever above 102.2°F, or for younger children with infections in both ears. Your provider will help determine the right approach.

A same-day visit is a good idea if your child has ear pain that does not improve within 1 to 2 days, drainage from the ear, fever, or persistent fussiness, especially when lying down.

Go to the emergency room or call 911 for severe headache, stiff neck, confusion, swelling or redness behind the ear, persistent high fever, or if your child is very lethargic. These can be signs of a more serious infection.

Walk-ins are welcome, and you can also reserve your time in advance for added convenience.
